Catholic Diocese of Pittsburgh Schools - Student Aid Form


Submitting this Student Aid Form will allow you to apply for assistance for your student(s) for the upcoming 2016-2017 school year for schools within the Catholic Diocese of Pittsburgh.

Application Submission Deadline

The deadline for application submission is March 15, 2016. The Pre-School application may be extended beyond this date. Please see your principal or program administrator.

The Catholic Diocese of Pittsburgh offers educational grants through the granting programs listed below:

Bishop's Education Fund (BEF)

This grant program is for Catholic students in grades K-12 attending eligible schools and registered in eligible parishes.

Opportunity Scholarship Tax Credit Program (OSTC)

This funding is available to families whose students reside within the attendance boundaries of a low-achieving school as determined by the Pennsylvania Department of Education and who meet the income qualifying guidelines. OSTC grants are for grades K-12 only.
You must provide a copy of the letter you received from the public school district confirming eligibility, answer the questions regarding the student, and complete and sign the OSTC Funding Supplemental Form. See second step below.

K-12 Scholastic Opportunity Scholarship Program (SOS)

This grant program is for any student in grades K-12 residing in Pennsylvania and attending any eligible Catholic school in the Catholic Diocese of Pittsburgh (see School Code List).

Pre-School Scholastic Opportunity Scholarship Program (SOS)

This grant program is for any Pre-Kindergarten student residing in Pennsylvania and attending any eligible Catholic school in the Catholic Diocese of Pittsburgh (see School Code List).
Proof of Residency and Pre-K Supplemental Application Required See first and second step below.

Other aid programs may be available through other scholarship organizations in cooperation with the Catholic Diocese of Pittsburgh

First Step

Review the list of documents that you will need to submit to PSAS. Note that some may not apply to your household.

  • You and your spouse's (if applicable) social security numbers.
  • A complete copy of your 2015 federal 1040, 1040A, or 1040EZ form, including all schedules and worksheets and copies of all 2015 W2 and/or 1099 forms for all individuals living in the household.
    • If you have not filed a 2015 IRS form 1040, please submit your 2014 Form 1040, 1040A or 1040EZ (as filed with the IRS, Including all Schedules) 2015 W-2 forms, 2015 1099, 1099R or 1098 forms for any wage earning adult residing with the applicants. If you are self-employed or own a business, rental property, and /or farm you will need to provide an estimate of this income for 2015. If this application is submitted after April 15, 2016, you must provide a copy of the 2015 extension for Filing Request, as approved by the IRS and a copy of your last filed tax return.
  • Your Social Services Year End Statement (AFDC, ADC, TANF) and/or Food Stamp Year End Statement and/or your Social Security Year End Benefits Statement(s) for ALL individuals living in the household receiving benefits in 2015.
  • Information about child support payments or child support received.
  • Information about the value and amount still owed on your home.
  • Information about the value and amount still owed on any real estate investments.
  • Information about the value of any savings or investment accounts.
  • Proof of residency: A copy of your 2015 Pennsylvania State Tax Return or a current utility bill (gas, water and electric are accepted).

Special Needs Families

If your student is a Special Needs Student, this form must be printed, completed, and signed by the parent and by the administrator at the school. This should be done prior to completing the application. The form must be uploaded when you complete and submit the application.

Families Interested in Applying for the Opportunity Scholarship Tax Credit Program (OSTC)

Opportunity Scholarship Tax Credit Funding will provide tuition assistance to families whose students reside within attendance boundaries of a public elementary or secondary school ranking in the bottom 15 percent of achievement as determined by the Pennsylvania Department of Education.

Complete the Parent/Guardian section in the form below, then please have the school principal or program administrator complete the yearly tuition rate for each student and sign the form. Then, include this form with your completed Catholic Diocese of Pittsburgh Student Aid Form to Private School Aid Service (PSAS). If you submit your application online, this form must be uploaded when you complete and submit the application.

Families that are Members of an Eligible Parish

Complete the Name of Student field in the form below, then please have the Pastor of your parish sign and date the form Then, include this form with your completed Catholic Diocese of Pittsburgh Student Aid Form to Private School Aid Service (PSAS). If you submit your application online, this form must be uploaded when you complete and submit the application.

EITC and OSTC Income Guidelines

Awards from the Catholic Diocese of Pittsburgh that are provided through the Pennsylvania EITC and OSTC programs carry the following income guidelines for eligibility:

Income guidelines are for EITC/SOS and Opportunity Scholarship Tax Credit Program (OSTC) funding only. These guidelines are subject to change.

Please Note: Qualification does not guarantee funding. Funding is distributed based upon the availability of funds.

Third Step

Complete the application.

As you progress through the application, if you have questions, simply click on the Question Mark Icon icon beside the question for a more detailed description of what is required. Do not leave any question blank. If a question does not apply to you, enter 0 or a similar answer.

PLEASE NOTE: You may only apply to one school per student. If undecided, apply to the school your student will most likely attend. If a change is required after the application is received by PSAS, you MUST notify the Department of Catholic Schools in writing.

Fourth Step

Verify the information that you entered

As you progress through the application, please review each page before moving on to the next.

Fifth Step

Submit the non-refundable $28 fee for processing your application. Payments via VISA, Master Card, American Express, Discover, or Electronic Bank Check (ACH) are accepted. All payment transactions are protected through the use of a secure server.

Final Step

Submit your application to PSAS for review by our staff.

You must submit the following required documentation via upload, fax or mail to complete the application process:

  1. A complete photocopy of your signed 2015 federal 1040, 1040A, or 1040EZ form, including all schedules and worksheets and copies of all 2015 W2 and/or 1099 forms for ALL individuals living in the household. If you have not yet filed your 2015 tax return, refer to the First Step for the list of documents you must submit with your application.
